Safety
Do I need to wear a mask to my appointment at Alaska Facial Plastic Surgery & ENT?
We suggest wearing a mask if you are sick; but patients do not otherwise need to wear a mask.
How many people can I bring with me to my appointment?
Minimize extra visitors during your visit. We recommend one parent accompany their child to their clinic visit and to leave siblings at home to reduce exposures during the COVID-19 pandemic.
What should I do I have a fever?
Individuals with a fever, cough, or shortness of breath, should stay home or contact their primary care provider’s office if they are concerned with possible COVID-19 exposure. Conducting Your Visit
What information should I bring to my appointment?
- Photo ID
- Medical insurance card
- Copies of any imaging studies you have had (the CD with your CT scan, MRI, or other study on it)
- Updated med list
- Other medical records related to the reason for your visit
What forms do I need to fill out?
A notification asking you to fill out your forms online will be sent to you once you book your appointment with us.
If I was referred to Alaska Facial Plastic Surgery & ENT, what should I bring?
It is helpful to being copies of any imaging studies you have had (the CD with your CT scan, MRI, or other study on it). Other medical records are helpful to bring if these explain your past medical history or reason for your visit. If you had an audiogram (hearing test), please bring a copy to review.
Billing
How much will medical services cost?
We can provide a basic quote for the consult but it is subject to change, due to the nature of a specialty visit. Some patients may require in-office procedures that might change/increase quotes.
We are happy to provide these procedure CPT codes for you to call your insurance company to confirm whether they are covered or not.
How much does a cosmetic consult cost?
Cosmetic consults are $100.00 and the fee is due at the time of service or in advance for a telemedicine consult. The fee of the cosmetic consult can often be applied to medical or procedural services offered.
The consult fee is due at the time the appointment is scheduled. If services are received same day, the fee will be applied to the treatment cost. If appointment is missed/no show, the consult fee is forfeit and not refunded.
How much do cosmetic procedures cost?
Once surgery is deemed necessary, you will receive a quote for the surgery based on procedures needed. This quote is for the surgeon only. You should expect to receive invoices for the surgical family and anesthesia, which are separate from our practice.
Costs for filler therapy (Juvéderm, Restylane), Botox therapy, Sculptra, and other services offered in the clinic are standardized and available on request.

How do I get help with payment?
You may be eligible for a payment plan or to use CareCredit for your visit. Please ask about this before your visit or at the time of service.

Who do I call if I have question and the clinic is closed?
Our providers are available after hours for urgent questions and emergencies. If our providers are out of town or unavailable, they will have a colleague available to answer questions. If you have urgent or emergent needs, go to the nearest Emergency Room.
